What to Expect

As a full service home health agency, we strongly believe in active communication with our clients and speaking with their families and caregiver to make sure we are meeting the customer’s needs and expectations. Patient satisfaction is strongly emphasized because not only do they need to be medically assisted, they also should feel comfortable. Upon first visit, we will have a registered nurse come and evaluate the patient’s primary needs based on their physician’s diagnosis. An assessment will be done based on the patient’s needs and situation. Our nurses will coordinate with the physician, to develop an effective care plan and will also coordinate with our office staff on the case. Depending on the evaluation, the skilled nurse may also request a physical therapist, occupational therapist, speech pathologist, or medical social worker to assist in your care. Throughout the care, we will continue to meet on a regular basis to monitor the program and make adjustments as needed.

A home health agency will provide home health care in 2 month periods, called episodes of care. After the 2 months end, if we believe that no further health care is needed then the patient may say homebound or they may be recertified for continued home health care.
